At the heart of Living Well is a dedicated Care Coordination Team focused on making sure you feel supported, informed and connected every step of the way. Acting as your main point of contact, the team works alongside you to plan, coordinate and manage your services, helping you get the most out of your Living Well experience at CRCS.
Led by Dominique Henss, Program Manager – Aged Care Services, Transport and Living Well, the Care Coordination Team includes Chisom Okoli, Proem Shrestha and Shiela Marie Dayola. Together, they take the time to listen, answer questions, manage service updates and resolve concerns, while working closely with all areas of Living Well — including Involve, Transport and Aged Care Home Services. With a strong person-centred approach, the team is here to ensure you feel heard, supported and confident in the care and services that help you live well.

Meet Dominique Henss
Domi has been part of CRCS for over seven years and brings a wealth of experience from supporting many different teams, including finance, NDIS, NEIST, Wellbeings and reception. She’s been with the Living Well team for the past 18 months and describes the team in one word: compassionate. For Domi, Living Well is all about supporting people’s day-to-day wellbeing—helping participants stay connected to their community, enjoy social activities, and live safely and confidently at home.
Outside of work, Domi loves getting creative with knitting and crochet and enjoys recharging by making music with her partner and kids as they learn new instruments together. She’s a fan of The Night Circus by Erin Morgenstern and loves hearing the life stories and experiences of older people in the community. Domi’s favourite part of working at CRCS is the strong focus on inclusivity for both participants and staff. What she believes makes Living Well special is the genuine passion behind the work and the strong person-centred approach, ensuring older people have choice, control and a real voice in shaping the support they receive.

Meet Chisom Okoli
Chisom has been part of CRCS and the Living Well team for just over a year, bringing a thoughtful and empowering approach to her role as a Care Coordinator. She describes the Living Well team in one word: empowering. For Chisom, Living Well is about finding the right balance between safety and independence—listening deeply to participants’ fears, goals and triumphs, and meeting them where they are emotionally as well as practically. It’s not
just about services, but about understanding what people want their day-to-day life to feel like.
What Chisom loves most about working at CRCS is the strong collaborative spirit. She works closely with rostering, transport, managers and participants to make sure no one falls through the cracks, always focused on getting things right for the individual. One of her most meaningful moments was supporting a participant who felt overwhelmed and frightened by care requirements; by taking the time to listen and walk through everything step by step, fear turned into confidence and security. Chisom is motivated by the small wins that lead to big improvements in quality of life, whether it’s better sleep, renewed pride in a garden, or simply feeling more in control. Her advice to anyone looking to connect with Living Well is simple: you don’t need a perfect plan, just a sense of what you’d like your life to look and feel like.

Meet Proem Shrestha
Proem has been part of CRCS and the Living Well team for almost two years and plays a key role in supporting people to feel connected, understood and supported. He describes the Living Well team in one word: caring—a team that genuinely supports one another and is committed to making a positive difference every day. For Proem, Living Well is about helping people stay independent, connected and confident, whether that means organising services, answering questions, or simply being someone participants can rely on.
Outside of work, Proem enjoys staying active and spending time outdoors, with hiking and playing football among his favourite pastimes. What he loves most about working with older people is hearing their stories, learning from their life experiences, and supporting them to remain engaged in their community. His favourite part of working at CRCS is seeing firsthand the positive impact the work has on people’s lives. Proem’s advice to anyone looking to connect with Living Well is simple—reach out for a chat. The team is always ready to listen, guide, and help find the support that’s right for you.

Meet Shiela Marie Dayola
Shiela has been part of CRCS and the Living Well team for just over two years and brings a deeply caring, holistic approach to her work every day. She describes the team in one word: a team—working together to support people in a truly person-centred way. At Living Well, Shiela focuses on putting each individual at the heart of their care, ensuring support is tailored to their goals, values, cultural background and identity, and delivered with respect and empathy so participants can live comfortably and confidently.
Shiela’s passion for working with older people is deeply personal. Growing up with her two grandmothers shaped her commitment to supporting older people with care, dignity and genuine connection. She finds it incredibly meaningful to listen to people’s stories, learn from their life experiences and build trust, ensuring they feel heard, valued and supported. Shiela is strongly motivated by seeing the positive impact of her work—helping people feel safer, more connected and in control of their choices. Beyond individual support, she
believes Living Well contributes to a more inclusive, compassionate and age-friendly community by reducing isolation and helping older people stay connected to their families and local networks.

Caring for an older loved one is rewarding and important, but everyone needs a break sometimes. Flexible respite, part of the Commonwealth Home Support Programme (CHSP), gives carers help so they can rest, recharge, or get things done knowing their loved one is safe and supported by caring, friendly and capable help at home or in the community.
Our Intake Team is often the first point of contact for older community members engaging with CRCS, and they play a vital role in shaping positive, supportive experiences from the very beginning. The team helps participants understand and access the wide range of care, activities and services available through our CRCS Living Well programs.
