To be eligible for a HCP, you must be over 65 years of age and have a completed My Aged Care assessment.
We also provide Community Transport support for members of our community with a Home Care Package. Ask your Package Manager to contact our Transport team on 02 6278 8124 or email email@example.com for a quote to deliver your required transport support.
